KIMBERLY CHAN, M.D., M.P.H. brings an extraordinary blend of clinical excellence, operational leadership, and deep commitment to innovation. Prior to becoming Era’s Director of Internal Medicine, she spent nearly two decades at Providence, one of the largest health systems in the country. Her roles there spanned Medical Director, Chief Population Health Officer, and Co-Chair of Southern California’s Medical Director Collaborative, all while actively caring for the greatest number of patients of anyone at this level of leadership within the network. During the pandemic, she was the clinical lead for all COVID operations for her medical group, led the broad rollout of telehealth and designed integrated care programs to close the gap between mental and physical health. She spearheaded major initiatives around blood pressure, post-discharge care, and patient-centric care models. As a native of Los Angeles, Dr. Chan has deep roots in the local care community. She trained at LA County–USC and has dedicated her career to building trusted relationships across the healthcare ecosystem. Her leadership helped make her division the highest-performing medical group across Providence for five consecutive years.
